My take is that when the model was first produced it was with suiside doors but
at some pont - I am assuming, the pattern was altered to show the doors opening in the more usual way. Speaking without any knowledge of this at all, but assuming that makers discovered that they modeled the doors incorrectly and once discovering this made a change to more accurately represent the real 1:1 ambulance. Hence the two ways the doors open.

The 1:1's were made both ways. It is known as three-way loading... by Lloyd Mecca #25497.2
